Physical description : 

Four-line mosaic inscription within a square frame. The first two letters ΠΡ are written as a monogram, and the word διακό(νου) is abbreviated by a small overhanging omega and a diagonal stroke on the leg of the kappa.

ύμων.

Translation: 

Offering of Conon, deacon of Jerusalem.
