☩ Πρωσφωρὰ ὁ̃ν Κύριος γινόσ-

κη τὰ ὀνόματα Ἅγιε Κωσμᾶ

(καὶ) Δαμηανὲ πρ(όσ)δεξε. Ἀμήν, Κ(ύρι)ε.

Translation: 

Offering of those whose names the Lord knows. Saint(s) Cosmas and Damianus, accept (it). Amen.
