Findspot: In the mosaic pavement at the opening of the apse in the chapel adjoining the church on the south.
Pres. loc.: The only part left in situ is the left-hand side of an incomplete triangle of the tabula ansata.
Fragmentary four-line mosaic inscription, beginning with a palm leaf, set within a tabula ansata. Frame (55 cm high, 320 cm wide) and calligraphic script similar to Inscription 2, but with greater chiaroscuro. Letters 10 cm high.
Ὑπὲρ [τελείας μνήμης καὶ ἀναπαύσ]εω[ς τοῦ ἁγίου]
πατρὸς [ἡμῶν Μαρτυρίου τοῦ ὁσιωτ(άτου) ἀρχιεπισκ(όπου) καὶ]
πατ[ριάχου τοῦ τῆς ἐν Χ(ριστ)ῷ συνοδίας συστησαμ(ένου) καὶ]
ἡγ[ουμένου ἐγένετο τοῦτο τὸ ἔργον.]
In [lasting memory and for the repose of our holy] father, [Martyrius, the most saintly archbishop and] Pat[riarch, founder and] heg[umen of this community in Christ, this work was done.]

Very little has survived of the inscription and the reconstruction highly conjectural.
